Responsibilities
- Provide direct support and assistance to individuals with disabilities in their daily activities
- Assist with personal care tasks such as bathing, grooming, and dressing
- Manage and administer medications according to individual care plans
- Accompany individuals on outings and appointments, ensuring their safety and well-being
- Promote independence and empowerment for individuals by encouraging and facilitating their participation in decision-making processes
- Maintain accurate and timely documentation of care provided
- Collaborate with other members of the healthcare team to develop individualized care plans and ensure effective communication and coordination of care
- Attend training sessions and staff meetings to stay updated on best practices and procedures
- Continuously evaluate and monitor the progress and needs of individuals, making necessary adjustments to care plans as needed
- Provide emotional support and companionship to individuals and their families, building trust and establishing positive relationships
Required skills, tools, and years of experience
- 12 months experience in a similar role
- CPR and First Aid certification
- NDIS screening check
